PADI Open Water Diver certification

A student diver should present the PADI Universal Referral form for Open Water Diver Certification (PDF) to a certified instructor. The form states that the diver received initial training at another diving school. This form instructs the instructor to complete the student’s open water dives and sign a statement. The form should then be sent to the original instructor who should certify it.

Although you can use the Universal Referral Form to obtain the certification, PADI insists that you complete this course within twelve months from your last training. It is typically a two- to three-day course. During this time you will need to be able to dive together to a maximum depth 60 feet/18m.

Medical statement

To become a certified scuba diver, you will need to complete a Medical Statement for Universal Referral. This document is vital and will make sure you are as healthy as you can before you dive. This document is meant to help you stay safe while diving, so be sure to answer every question honestly.


First, ensure that your doctor signs the form. This form must be signed by your physician before you begin training to become a scuba diver. A medical release is required for any new medical conditions. A photocopy of your certification card is recommended.

Logbook

Before a diver is allowed to dive open water, they must sign a medical form. This usually happens before certification classes begin. The medical release must be signed by a physician. You will need a new release if your medical situation changes. PADI allows you to obtain a medical waiver. This medical release is equivalent in size to the PADI Liability Release/Assumption and Risk Form - General Training, or the EU version.

The universal referral is different from that used in a diving logbook. It will list the different open water dives and knowledge development sections that the student completed. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student is then required to take the form back to their instructor to be certified as a diver.

Completion requirements for courses

Most well-known scuba certification agencies participate in the Universal Referral Program. This means that a diver can take the theory and pool part of his or her open water course with his or her local dive center but complete the open water check-out dives with a different agency. This can only be done in cooperation with a dive centre that is a member of a universal referral programme such as SSI (NAUI), NASDSS or YMCA.

PADI Open Water Divers requires that students pass the Knowledge Development as well as Open Water Dives sections. A medical release must be signed by the student diver before he or she can dive. To be certified, the student diver must take a photo. Digital images can be accepted.
